Steady on: Helping Seniors Stay on Their Feet

As people age, the risk of a fall turning into a debilitating injury increases exponentially. Michael Godard, assistant professor of sports medicine, is doing something to help senior citizens stay steady on their feet.

Godard is heading a research project, based in the athletic training lab in Gorham, which uses feedback generated by sophisticated machinery to ultimately lead subjects to increased balance and stability. The subjects-eight so far-are all over 75, are not obese, and have no major musculoskeletal abnormalities. They volunteered to participate in the study, which is now in its eighth week. >>

It has been suggested that while they may have enjoyed lucrative compensation packages or hall-of-fame careers, some of these athletes paid a high price with irreversible damage to their health. The epidemic that is concussion in sports continues to affect many of us and has trickled down from the professional and college fields to the youth sports that many of us played. The majority of physical therapists will not have the opportunity to work with the post-concussed professional or collegiate athlete, but all of us can positively affect the incidence of concussion in youth sports leagues through education, comprehensive baseline concussion testing, and post-concussion management as rehabilitation specialists. The technologies available to us in this area of practice continues to grow and evolve. >>

Much of the conversation around concussions in youth sports focuses on collision sports such as football, hockey and lacrosse. Little attention is given to the athletes on the sideline, cheering their teams to victory.

“If you can maintain a minute, you’re doing pretty well with balance,” said Erin Smith, wellness director at Westhills Village Retirement Community. If you wobble before time’s up, don’t panic. With age comes wisdom, white hair — and wobbles. “Unfortunately, as we age, our balance is one of the first things that we tend to lose,” Smith said.

New Millcreek clinic treats concussions

Zaihe Regus closed his eyes and stood still on a nearly 2-inch-thick piece of foam.

The Mercyhurst University freshman's feet swayed slightly on the cushion, which is part of the Biodex Balance System. It measured how well Regus, 18, kept his body in balance.

"It's like I'm standing on Jell-O," Regus said.

Regus underwent the test Friday at the Concussion Clinic at the Lake Erie College of Osteopathic Medicine Medical Fitness & Wellness Center, 5401 Peach St.

Glenwood is a skilled nursing facility that provides short-term rehabilitation as well as long-term care. Tool helps monitor kids' concussions

COLCHESTER, Vt. - Some schools like Colchester High School conduct baseline concussion screening tests for each athlete at the beginning of the school year. It is not a state requirement. There is some debate over the accuracy of the computer-based tests, which is why some parents are opting to have their children tested independently.

"It is a big issue because you need to make sure your athletes can return to what they did before. If you don't get a baseline test and they get hit in the head, you don't know where they are starting from in terms of making a return to play decision. So you need that to understand are they back to a baseline or for us within 15 percent of their baseline, so they can be cleared to go back into sport," said Sean Fitzgerald, a physical therapist.

Transitions Physical Therapy uses a test that includes a review of symptoms, neuro-cognitive processing and something called a neuro-physical balance assessment, using a machine called the Biodex Balance System. >>

ELLSWORTH — Falls are common among people over 65. The Centers for Disease Control and Prevention has identified falling as the top cause of injury-related deaths in this age group. Falls affect about 30 percent of seniors and trigger 1.8 million injuries annually.

Elderly people’s risk of falling, however, can be effectively reduced through various interventions. All of Hancock County’s hospitals are equipped to properly assess an individual’s balance and risk of taking a tumble. Noteworthy, though, is the fact that Maine Coast Memorial Hospital staffers campaigned for and served as guinea pigs in the development of a portable balance-testing and training device enabling them to provide those services at all their hospital’s rehab sites including Ellsworth, Somesville and Gouldsboro. >>

BETHLEHEM, Pa. – Balance is something most people take for granted and nobody knows that more than Chris Henry of Palmer Township.

For the last few years she has had serious problems with her balance and couldn't get much help from doctors.

"It's not frustrating, it's devastating," she said. "I have to hold the wall all the time because when you walk you feel like walking on a cloud," she recalled about what it used to be like for her.

"I was falling on the floor unconscious from taking showers," she said. Henry was at her wits' end and said she even got depressed because nobody was able to pinpoint what was wrong and offer a solution. >>

Aurora Sports Medicine Institute (ASMI) is the sports medicine branch of Aurora Sheboygan Memorial Medical Center, in Sheboygan,  WI. Rehab Equipment Gets Patients Back on Track

Bolingbrook resident Richard Bonham, 72, was enjoying his retirement, including working on projects at his home workbench. But his comfortable routine was interrupted on March 17, 2012, when a stroke impaired his ability to walk, the use of his left arm and his vision. He also suffered several fractured bones during subsequent falls.

After stints as an inpatient at a couple of rehabilitation facilities, Bonham continued his rehab in October as an outpatient at Edward Hospital.

“Happily, my vision had vastly improved, but I had to learn to walk again,” he said.

Bonham’s timing was good – Edward’s Rehabilitation Department had just installed new state-of-the-art equipment to help patients with mobility and balance issues.

“We’re one of the few area hospitals with a partial body weight support system to help patients re-learn to walk,” according to Karen Granato, PT, ATC, OCS, Edward’s Director of Rehabilitation Services. “This involves hooking the patient into a harness suspended from a ceiling track, which runs over the treadmill and around the room. The therapist controls how much weight the patient needs to bear, making it easier to walk.”  >>

Weight Transfer Tops Reasons Seniors Fall

The most common reason older individuals fall -- at least among those living in long-term care facilities -- is the incorrect shifting of body weight, researchers found.

Of 227 falls captured on video, 41% were related to incorrect transfer or body weight shifting, Stephen Robinovitch, PhD, Simon Fraser University in Burnaby, British Columbia, and colleagues reported online in The Lancet.

Other common causes included trips or stumbles (21%), hits or bumps (11%), loss of support with an external object (11%), and collapse or loss of consciousness (11%). The least common cause was slipping (3%).

"By providing insight into the sequences of events that most commonly lead to falls, our results should lead to more valid and effective approaches for balance assessment and fall prevention in long-term care," the authors wrote. >>

Balance Exercise Helps Those with Impaired Vision

Gait and balance of patients with Diabetes can be improved: Specific training (balance exercises) can improve gait speed, balance, muscle strength and joint mobility in diabetic patients.
 
Gait, balance, fear of falls, muscle strength and joint mobility were measured at baseline, after intervention and at 6-month follow-up.
RESULTS: After training, the intervention group that were given balance exercises increased habitual walking speed by 0.149 m/s compared with the control group. Patients in the intervention group also significantly improved their balance (time to walk over a beam, balance index recorded on Biodex Balance System), their performance-oriented mobility, their degree of concern about falling, their hip and ankle plantar flexor strength, and their hip flexion mobility compared with the control group. >>

Tallahassee, Florida - The future of sports science takes a leap forward with a new laboratory at Florida State University.

The university unveiled its Human Performance Laboratory on Friday. Sports research teams will study all kinds of variables linked to athletic performance, such as speed, strength, agility, body composition and balance.

The lab uses specialized equipment to measure those things. A stationary bike equipped with sensors and linked to a computer measures how many watts a cyclist can put out in a burst of acceleration. That information shows exactly how much power an athlete can generate over a period of time.

The lab also has a machine called the Biodex Balance System, which precisely measures someone's ability to balance. The equipment can be an important tool for an athlete recovering from a concussion. >>

A study has been released stating that young athletes suffering head injuries can risk death returning to action too soon. None of the protective gear worn by athletes can keep you completely safe – a second hit to the head can develop catastrophic swelling. Consider establishing unified standards, for example a sideline exam with a Biodex Balance device, so that it’s not the player’s decision and maybe not even the coaches decision to determine if the athlete should return to play.

Sue Fulgham has had more than her share of physical ailments.

She has had three hip replacements, two broken femurs, knee replacement and a ruptured disc in her back. In June, she suffered a fall that left her with a broken femur.

"If my leg had gotten infected, I would've lost my leg," the Logan County woman said. "I was in a hospital bed all summer."

The fall also left her with a slightly off-balance gait.

"I leaned forward and to the side. My balance was off," said Fulgham, tilting her hand to the right to show the way her body leaned. "I started physical therapy in September."

About three weeks ago, she started working on a new machine called the Biodex Balance System at Bluegrass Outpatient Center. The facility received it about a month ago. >>

Balance is Crucial to Avoiding Falls after Surgery

I had a hip replacement four weeks ago, and I'm happy to say my doctor said the surgery went exceedingly well, and I'm already walking around with no problem. Also on Monday I started outpatient physical therapy at a nearby rehabilitation facility — a rigorous bunch of exercises including using a recumbent bike and various dumbbells and ankle weights.

One exercise involves trying to stand on one foot for a minute. (I challenge you to do it.) This has to do with the all-important sense of balance.

In order to walk safely it's important to put equal amounts of weight on each foot. But most people have altered their balance before they have a knee or hip replacement because of the pain in the subject hip or knee. And they are at an even higher risk of falling after they have a new hip or knee because it's painful to put pressure on the new joint. >>
